How do I make Media Encoder generate an h.264 .mp4 file for 720p @ 60fps?

I have a bunch of source material at 720p, 60fps, that I'd like to encode into a format I can actually play on a computer. The only option I see is "h.264 Blu-Ray" which generates a bare m4v stream for burning onto a Blu-Ray disc, rather than a format that works out of the box: how do I get Media Encoder to give me a format I can actually use?

Picking h.264 with "Match Source - High bitrate" yields 720p at 30 fps, rather thatn 60 fps, so that's not an option.
